What You'll Need

Upon your first visit to Dr. Scott you will need to bring a couple things with you.  Please bring your insurance card as well as photo I.D.

You will also need to bring one of the below packets with you to the first visit. Simply fill in the information and either print or email them to the office at admin@drscottosteo.com  If you are seeing Dr. Scott for a Motor Vehicle Accident please select the New MVA Packet.  For all others please use the New Patient Packet.
